The Massachusetts Department of Public Health requires all full-time undergraduate and graduate students under 30 years of age, all health science students, and all international students, to submit immunization records to their enrolled college or university regarding Tdap, MMR, Hepatitis B, Meningitis, and Varicella vaccines.

Note that Suffolk University recommends Meningitis B vaccine, Seasonal Influenza, and COVID vaccine as well.

  How to submit your records?

To submit these records, you will need to upload your documents through the Suffolk University Care Health and Wellness department’s Student Health Portal. Please click the button to access the site.

Once you have entered the student health portal you will see this welcome message. Enter the date of your birth to continue.

You will see various options for you to choose.

Click forms and requirements. Then proceed to update your choice of immunization record by clicking Update. In some cases, they might ask for documents upload.

Alternatively, you may schedule for an appointment with CHW Staff.

By choosing this path, you must click No, unless you are experiencing health emergencies.

Click Immunization Visit to proceed with the appointment.
