Apostille Process Request Form
An Apostille is a certification issued by the Commonwealth of Massachusetts that verifies the authenticity of a public
document being presented to a foreign country that is a signatory to the Hague Convention of 1961. The Office of the
Registrar will authenticate, notarize and make available the Apostille process to students who need to certify a diploma
and/or an official transcript.
Change Of Address
Please complete this form and submit to the Registrar’s Office if your permanent address has changed while you are a Suffolk student.
Change of Major/Minor
If you are changing you major to Political Science and/or Legal Studies, please complete this form and email to PSLS administrative coordinators Jeff Fish or Elise St. Esprit who will assign you an advisor and pass the completed form to the Registrar’s Office.
Change of Name
Please complete this form and submit to the Registrar’s Office if your legal name has changed while you are a Suffolk student.
Enrollment Verification Form
Excess Course Petition
Fill out this form to petition for excess credit hours in a semester. Students can take up to 17 credit hours per semester.

Independent Study Requests

Once this form is filled out, along with an Independent Study proposal form below, please email them to department coordinator Elise Saint-Esprit at estesprit@suffolk.edu who will process it and send it to the Dean’s office for approval.

In addition to the Independent Study Request Form above, you must work with your professor to essentially build a syllabus that includes readings, grading/assessment, a course description, the number of credits, and the number of hours you will you will spend according to the number of credits.
Incomplete Contract
Sometimes, for a variety of reasons, students are not able to complete their coursework during a semester. If you are struggling in a course, the first step is to reach out to your professor and see what your options are. From there, if your professor agrees to let you take an incomplete and finish it later, your next step will be to fill out an Incomplete Contract. Please make sure to keep a copy for both you and your professor.
If you take an incomplete, you generally have a year to complete the coursework before the grade turns into an “F.” However, your professor has the right to require you to finish sooner. This contract is to make sure you have a clear plan for finishing your course and to keep a record of the agreement you have made.
In the event that you cannot finish your incomplete within a year, you have the option to discuss extending your Incomplete Contract with your professor. If you both agree to an extension, you will then fill out an Extension of Incomplete Form.

Pass/Fail Request Form
A Pass/Fail option is available to undergraduate students in lieu of a traditional letter grade provided it meets the guidelines
listed below:
- The pass/fail option is available to undergraduate students in good academic standing who have completed a
minimum of 15 semester hours of coursework at Suffolk University. - The pass/fail option is only applicable to elective courses and may not be used for courses that fulfill general college,
core, major or minor requirements. - The pass/fail option is limited to 12 credits for the duration of your degree program. Courses taken Pass/Fail during
the Spring 2020 term do not count towards the maximum 12 credits of pass/fail courses which can be taken.

The Department of Political Science & Legal Studies recognizes the importance of student scholarly work and accomplishments. Our Student Research Conference (SRC) is an annual opportunity to undergraduate and graduate students to present the research they have conducted. Upon acceptance, students at all academic levels, including first year students and graduate students, may present their scholarly work at the conference.
